How to Register
We are excited you are interested in participating with Portneuf Valley Adaptive Sports. First of all, we need to learn a little about you. The following information will help us understand your abilities as well as some challenges you may have.
It will help us figure out what adaptive equipment if any you might need. We may contact you before your activity if we have any specific questions after reviewing the information given. Please feel free to contact us if you have any questions.
First Time Participants:
Fill out the New Participant Registration & Eligibility Criteria by clicking on the Register Now Button.
Fill out the Activity Request Form and sign the Acknowledgement of Risk and Liability Release by clicking on the Activity Request Button.
Returning Participants:
Returning participants can go directly to the Activity Request Form and sign the Acknowledgement of Risk and Liability Release by clicking on the Activity Request Button.
Activity Requests
Currently as a new program, PVAS has limited equipment resources and instructor base. PVAS only provides instruction and adaptive equipment for the activity.
Any personal assistance or special needs is the responsibility of the participant. Those wanting to participate in a PVAS activity should submit a request at least 2 weeks in advance to give us time to figure out if we have the resources available to make this happen.

PVAS Essential Eligibility Criteria
Companions
Companions are people who are very familiar with the participant and can assist the participant with physical, emotional and or communication needs. They have the ability to take care of the participants personal needs.
Trained Companions
Trained companions provide special needs for the participant such as administering medication, handling any medical issues or helping with communication such as signing for a person who is hearing impaired.
PVAS programs and activities are for individuals requiring specialized instruction or specific equipment adaptations.
Activity Request Information Risk Acknowledgement & Liability Release
As a PVAS participant...
I can manage personal care independently or with the assistance of a companion.
I can signal others if I am distressed, injured or need assistance independently or with the assistance of a companion.
I can follow and understand instructions, recognize hazards and manage risks independently or with the help of a companion.
I understand if medication needs to be administered in the event of a seizure and/or any other medical condition that requires immediate administration of medication or attention, a parent/guardian or trained companion must accompany me throughout the program.
I can contribute to a safe learning environment. I can refrain from behaviors that pose a risk to myself or others. These behaviors include but are not limited to aggression, inability to set boundaries, a lack of safety awareness and drug or abuse independently or with the help of a companion.
